THE JEWISH FEDERATION OF METROPOLITAN CHICAGO
Qualifications
The primary criteria for an award are career promise and financial need. Please review the following eligibility requirements before proceeding to the application instructions and forms.
1. Applicants must be Jewish
2. Applicants must meet one or both of the following criteria:
1. Born and/or raised in either:
Cook County, Chicago metropolitan area, or Northwest Indiana
OR
2. One continuous year of full-time employment in Cook County or Chicago metropolitan area prior to starting professional education.
PLEASE NOTE: The Chicago metropolitan area includes Cook, DuPage, Kane, Lake, McHenry and Will Counties. Northwest Indiana includes Lake County.
3. Applicants must intend to remain in the Chicago metropolitan area after completing school
4. Applicants must be entering an accredited professional graduate education program on a full-time basis (school acceptance is not required at time of scholarship application)
5. Undergraduate applicants must be entering the junior or senior year of an accredited professional education program that will allow them to pursue employment in their chosen helping professions at a Bachelor's degree level
6. Applicants must demonstrate career promise in a helping profession such as: Medicine, Social Service, Performing and Fine Arts, Social Service, Business, Healthcare, Law, Technical/Sciences.
(please check with JVS at (312)673-3444 to verify "helping profession" eligibility before completing application)
7. Applicants must be available for a personal interview at Jewish Vocational Service between March 1 and April 20, 2010.

Submit
1. Review eligibility requirements carefully.
2. Download and complete all forms (see Academic Scholarship Menu):
* Legal Domicility Form
* Applicant Data Form (New or Renewal)
* Career Statement (New or Renewal)
* Letter of Reference Form (providing two professional and/or academic references)
* Financial Summary
* Release of Information Form
3. When filling out forms, edit your responses accordingly:
* Font size must be no smaller than 10.5 and no larger than 12
* Stay within and do not enlarge margin lines
* Do not use any bold font
4. To begin the application process, complete and send the following forms as a Word document via e-mail attachment to: jvsscholarship@jvschicago.org.
* Applicant Data Form (New or Renewal)
* Career Statement Form (New or Renewal)
* Financial Summary
5. Complete and send the following forms/documents via regular mail to: Scholarship Secretary, Jewish Vocational Service, 216 West Jackson Blvd., Suite 700, Chicago, IL 60606. You can submit these forms separately as they are available.
* Legal Domicility Form
* Two letters of professional and/or academic reference (renewal applicants may omit)
* Applicant IRS Forms filed for 2008 and 2009 when completed
* Parents of students under the age of 26 and/or spouse's IRS Forms filed for 2008 and for 2009 when completed
* Documentation of tuition costs, fees and student loans
* Release of Information Form
* Official academic transcript (if in school, from present school; if not in school, from most recent school attended)
