The fundamental problem here isn't just the pharmacist's screw-up. Had the technician received even rudimentary medical and A&P training, she would have known that a sodium concentration of 24% is a HUGE no-no, and not just that "something is wrong." The mistake shouldn't have occurred to begin with.
